Provides outreach, engagement, and navigation services for individuals experiencing homelessness. Services may include: Behavioral and mental health support Wrap-around case management Outreach and engagement with individuals experiencing homelessness Needs assessment and vulnerability screening using OneHome Assessment Referrals to community services Coordination with law enforcement to connect individuals to services instead of emergency rooms or jail Services: Housing Related Coordinated Entry, Mental Health Crisis Lines, Transitional Case/Care Management
