Who can use it

Individuals must: Be aged 16 and younger at the time of application Be a U.S. Resident receiving treatment in the U.S. Have primary coverage through a commercial health plan, with Medicaid or CHIP allowed as secondary coverage Be under the care of a licensed medical professional and applying for prescribed treatments, equipment, or services Meet the following family income limits, as documented on IRS Tax Form 1040: $65,000 or less for a family of 2 $100,000 or less for a family of 3 $135,000 or less for a family of 4 $170,000 or less for a family of 5 or more
