Provides referrals and assistance connecting individuals and families in Huerfano and Las Animas counties to available community resources, including: • Housing assistance • Food resources • Employment search support • SSI/SSDI application assistance • Mental health services • Substance use disorder services Additional referrals may be provided based on individual needs.
