This program aims to: Identify those who may qualify for disability benefits Walks clients through the eligibility process required to access Colorado’s Home and Community-Based programs Provide support in accessing the appropriate wraparound services Connects clients with the individualized support they need to pursue a future away from homelessness Services: Residential Placement Services for People With Disabilities
