Supported Living Service

This service is a Medicaid Waiver Program for individuals with developmental disabilities. State funding is available for those without Medicaid. The program provides person-centered support to help individuals remain in their homes, live with family, and participate in the community while maintaining independence. Services include: Personal Care Supported Community Connector Homemaker Basic and Enhanced Dental/Vision Mentorship Assistive Technology Respite Day Habilitation Services and Supports Vehicle Modifications Behavioral Services Home Accessibility Adaptations Specialized Medical Equipment and Supplies Services: Supported Living Services for Adults With Disabilities
